I am collecting the following information to construct a model to show operating costs for the 2015-2016 school year.

As most of you know, I have collaborated with Dan Gottlieb on several aspects of analyzing the closure. I recognize some of the information I am requesting is extremely sensitive in nature, and due to the sensitive nature of it, I will not be sharing individual details with him.  I have included a check box to allow individual detailed information to be shared with the college and the legal teams should you so desire.

I am asking for compensation information, but it is not a required field.  Knowing who will return will be helpful, but knowing the salary expense is a very important metric for determining viability.  Again, your individual information will be kept completely confidential unless you specifically authorize me to release it, otherwise information will only be released with aggregate total faculty compensation.
