Data request for all current and accepted Sweet Briar students

To all Sweet Briar students and those who hope to be Sweet Briar students for the 2015-2016 school year:

Data is being collected for the purpose of building a financial forecast to determine operating revenue and expenses of Sweet Briar College.  It will be used to show that the college is viable going forward.  I am asking all current students and those hoping to be entering freshmen or transfers to complete the form.

Unless you authorize the release of this information to the college and the legal teams involved in all of the lawsuits below, your detailed information will be kept confidential. It is preferable that the information be authorized for release.  In all cases the data in the last field of the form will not be shared with any other party.
